Compensation: $115,000 - $135,000 per year ( based on experience)

Job Description:

Are you a dedicated Provider seeking a flexible role where you can make a meaningful impact on patients' lives? Focus Care Inc. is looking for experienced Nurse Practitioner & Physician Assistant to join our team in conducting in-home health assessments.

As a Provider with Focus Care, you will:

• Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions.
• Provide education on medication management, disease processes, and preventive care.
• Identify and document diagnoses to support care management and active treatment.
• Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
• Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.

Minimum Requirements:

• Valid Nurse Practitioner or Physician Assistant license in the state of Maine .
• Board Certification from AANP, ANCC, AACN, NCCPA
• Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
• Strong communication skills and a commitment to delivering compassionate care.
• Reliable transportation for travel within the assigned areas.
• Ability to manage various dwelling conditions and engage with patients comfortably in their home environments.
